LPN Acute Care Admission/Discharge Nurse
Licensed Practical Nurse - Acute Care Admission/DC Nurse
GENERAL SUMMARY:
Under the direction of a Registered Nurse, the LPN will provide direct patient care, meeting both the psychological and physical needs of the patients in accordance with physicians’ orders, JCAHO, OSHA, nursing, and department guidelines. The LPN fulfills these duties to contribute to the continuity and quality of care that fosters the best interest and well-being of all patients and their families.
ESSENTIAL JOB DUTIES:
Act as an Admissions/Discharge Nurse as directed by the Registered Nurse.
Document skin assessment/wound daily in all patient medical records and communicate with the patient’s provider or under the direction of the Registered Nurse for treatment of wounds if needed.
Utilize manual and automated information systems to improve efficiency and effect productivity.
Conduct and document a thorough assessment of each patient’s medical status upon admission and throughout the patient’s course of treatment.
Under the supervision of the Registered Nurse, assist in the implementation of the patient’s treatment plan.
Complete required documentation in an accurate and timely manner.
Provide regular patient status updates to appropriate personnel.
Attend and participate in department/facility meetings, as required.
Perform other duties, as assigned.
MINIMUM KNOWLEDGE, SKILL, AND ABILITIES REQUIRED:
Excellent communication and interpersonal skills.
Must have current knowledge of medication action, administration, side effects, and contraindications.
EDUCATION AND EXPERIENCE REQUIRED:
Valid license as a Licensed Practical Nurse in the State of Kentucky.
Certified in CPR.
